Your Right to Receive a Good Faith Estimate
Under the law, healthcare providers must give patients who do not have insurance or who are not using insurance an estimate of the expected charges for medical services.
This is called a Good Faith Estimate.
What Is a Good Faith Estimate?
A Good Faith Estimate (GFE) shows the expected costs for services and items you may receive, such as office visits, lab work, or treatment plans.
It is not a bill. The actual cost may vary if additional services are recommended or required during your visit.
Who Is Eligible
You are entitled to a Good Faith Estimate if you:
- Do not have health insurance, or
- Do not plan to use your insurance to pay for your care at Spruce Health Collective.
When You’ll Receive It
- If your appointment is scheduled at least 10 business days in advance, you’ll receive your GFE within 3 business days.
- If your appointment is scheduled at least 3 business days in advance, you’ll receive your GFE within 1 business day.
- You can request a GFE at any time, even if you haven’t scheduled yet.
What’s Included
Your Good Faith Estimate will include:
- The estimated cost of the primary service (such as a naturopathic consultation or acupuncture session)
- Any related items or services that are reasonably expected as part of your care plan
- The provider’s name and contact information
- The date and service description
If the Bill Is Higher
If you receive a bill that is $400 or more above your Good Faith Estimate, you may dispute the charges.
To start a dispute:
- Contact your provider first to review the bill.
- If unresolved, you can file a dispute with the U.S. Department of Health and Human Services (HHS) within 120 days of receiving the bill.
There is a $25 filing fee, which may be refunded if the review finds the billed amount was incorrect.
